Caregiver Navigation Program is based on the Adult Day Services Plus (ADS Plus) program which is an evidenced-based program to help caregivers navigate the caregiving journey. 

Throughout this program, you will:

    • Participate in a caregiver-directed program to prioritize the unmet needs and wellbeing of caregivers

    • Receive one-on-one care tailored to your unique circumstances

Caregivers enrolled in the Caregiver Navigation Program experience:

    • Enhanced knowledge and skills of caregiving

    • Improved quality of life

    • Reduced caregiver stress

    • Increased confidence as a caregiver

What is Caregiver Navigation?

    • Helping caregivers access the right services at the right time

    • Assisting caregivers to gain access to home and community based services

    • Facilitating connection with caregivers through direct support

    • Ensures caregivers feel supported and informed of services and supports within the organization and the community

Respite care – limited scholarships are available for adult day services at The Life Center

The Caregiver Navigation Program

This program provides family caregivers with person-centered collaborative support. Our Care Managers provide comprehensive assistance that is responsive and individualized to each caregiver.

With this flexible approach to caregiving, you will meet with a Care Manager to navigate personal care challenges at times that are convenient for you over the course of the program.

You will become actively involved in the planning and provision of care, while recognizing your preferences and needs as a caregiver.

During the enrollment process, you will integrate yourself as care partner to improve your quality of life, as well as the care recipient’s quality of life.
