or 6 months of Day Care Service
$1,000 - 18 days of Day Health Care,
or 23 days or Day Care Service (1 month)
$500 - 9 days of Day Health Care,
or 12 days of Day Care Service (1 week)
$100 - up to 2 days of Adult Day Health Care,
or 2 days of Adult Day Care Service

Hours of Operation:
Monday through Friday,
7:15 a.m. through 5:30 p.m.
(hours and number of days in
attendance are flexible)
Fees: (determined by Level
of Care):
Level I: $40.00 per day
Level II: $45.00 per day
Level III: $55.00 per day
(grants/scholarships availability is limited)
The Life Center Provides:
- nutritious meals and
special diets
- medication management
- health care monitoring(includes
blood pressure/blood sugars)
- bowel and bladder retraining
- a safe and secure environment
- socialization and friendships
- reasons to get up each
day
- unconditional acceptance
- physical activity and
exercise
- a full-time Registered
Nurse and professionally trained staff
- a state certified facility
- a monthly Alzheimer’s/Dementia
Support Group
- the educational series
